🏥 Hospital जाने से पहले ये 15 चीजें जरूर तैयार रखें

1. 🪪 ID Proof
   सबसे पहले अपना valid ID proof रखें।
   जैसे:
   Aadhaar Card
   PAN Card
   Driving Licence
   Passport या अन्य accepted ID
   Hospital registration और admission के समय इसकी जरूरत पड़ सकती है।
2. 📄 Previous Medical Records
   अगर patient पहले किसी बीमारी का treatment ले रहा है, तो उसकी पुरानी reports और medical records जरूर साथ रखें।
   इसमें शामिल हो सकते हैं:
   Previous prescriptions
   Discharge summary
   Blood test reports
   X-ray
   CT/MRI reports
   ECG
   Echo reports
   अन्य relevant medical documents
   पुरानी reports doctor को patient की medical history समझने में मदद कर सकती हैं।
3. 💊 Current Medicines की List
   Patient अभी कौन-कौन सी medicines ले रहा है, इसकी एक updated list बनाकर रखें।
   इसमें लिखें:
   Medicine name + Dose + कितनी बार + किस समय
   अगर possible हो तो medicines के original strips/containers भी साथ रखें।
   Important: Doctor की सलाह के बिना medicines बंद या dose change न करें।

   अगर patient को किसी medicine, food या अन्य चीज से allergy है, तो इसकी जानकारी family members को भी होनी चाहिए।
   एक simple note बनाकर रखें:
   “Patient is allergic to __.”
   यह information healthcare team के लिए बहुत important हो सकती है।
5. 🏥 Admission / Doctor Details
   अगर admission पहले से planned है, तो:
   Hospital का नाम
   Doctor का नाम
   Department
   Appointment/admission date
   Admission time
   Contact number
   एक जगह लिखकर रखें।
   Emergency में information ढूँढने में समय बर्बाद नहीं होगा।
6. 💳 Insurance / Health Card
   अगर आपके पास health insurance, government health scheme या hospital health card है, तो उसे साथ रखें।
   Insurance policy details और TPA-related documents भी जरूरत के अनुसार रखें।
   अगर cashless treatment है, तो hospital से पहले ही required documents और process confirm कर लेना useful होता है।
7. 📱 Important Phone Numbers
   Patient के साथ रहने वाले family members के phone numbers save होने चाहिए।
   कम से कम:
   Primary family contact
   Alternate family contact
   Doctor/hospital contact
   Insurance/TPA contact, if applicable
   Emergency contact information एक paper पर भी लिखकर रखें।
8. 💰 Payment Method
   Hospital admission में registration, deposit या अन्य charges की जरूरत पड़ सकती है।
   इसलिए:
   Debit/Credit Card
   UPI
   Cash, if required
   Insurance details
   का arrangement पहले से रखें।
9. 👕 Comfortable Clothes
   Hospital stay के लिए comfortable और loose clothes रखें।
   साथ में:
   Nightwear
   Undergarments
   Socks
   Slippers
   Personal hygiene items
   रखना useful हो सकता है।
10. 🧼 Personal Hygiene Items
    कुछ basic personal items साथ रखें:
    Toothbrush
    Toothpaste
    Soap
    Comb
    Towel
    Tissue
    Hand sanitizer
    Hospital में stay कितना लंबा होगा, यह हमेशा पहले से पता नहीं होता।
11. 👓 Spectacles और Hearing Aids
    अगर patient glasses, hearing aid या कोई other assistive device इस्तेमाल करता है, तो उसे साथ ले जाना न भूलें।

12. 📱 Mobile Phone + Charger
    आज के समय में mobile phone बहुत useful है।
    Family members से communication, hospital updates और important information के लिए phone की जरूरत पड़ सकती है।
    इसलिए:
    Phone + Charger + Power Bank (यदि उपलब्ध हो)
    साथ रखना practical है।
13. 📝 Important Medical Information
    एक छोटा सा note तैयार रखें जिसमें patient की basic medical information हो:
    Name → Age → Medical Conditions → Current Medicines → Allergies → Emergency Contact
    अगर patient emergency में खुद information नहीं दे पा रहा है, तो यह note healthcare team के लिए helpful हो सकता है।

    Hospital जाने का मतलब पूरा घर साथ ले जाना नहीं है।

    Emergency situation में सबसे पहले patient की medical safety important है।
    अगर patient को severe chest pain, difficulty breathing, unconsciousness, severe bleeding, stroke के symptoms या कोई अन्य serious emergency है, तो सामान pack करने में समय बर्बाद न करें।
